Spot the Spot is a free community event offering skin cancer screenings, sunscreen, lemonade, door prizes, and sun safety education for neighbors of all ages.

Screenings are limited and educational. They are not a replacement for a full medical exam, diagnosis, or ongoing care from your health care provider.

I understand that Spot the Spot provides limited, educational skin cancer screening services at a community event. This screening is not a full medical examination, diagnosis, treatment plan, or substitute for care from a licensed health care provider.

I acknowledge that follow-up medical care is my responsibility and that I should contact a qualified provider about any skin concern, urgent symptom, or recommendation made during the event. I voluntarily participate and release event organizers, volunteers, sponsors, and site partners from claims related to participation, except where prohibited by law.

I consent to basic event communications related to my waiver and understand that screening availability is limited.
